Abstract: This case study attemptedto find out the causes of failure in mathematics inSLC in an ineffective secondary school. This study was donein one of the lowestperforming secondary school of Dailekh district the research was held on the basis offollowing objectives :  To find out the causes of failure in mathematics in SLC in an ineffectiveschool of Dailekh district.  To find out the effectof causing variables on low performance of students inineffective school. To meet these objective, teacher, SLC failed students and Head teacher werechosen as respondents. For this purpose the researcher selected 10 students (40%)among 25 exempted candidates for sample by using systematic random samplingmethod Interview, observation and school documents were used to collect data andinformation these were categorized according to different factors contributing failurein mathematics. Different theoriesand models were used for analysis andinterpretation of data. This case study has focused on three variable s and its influence onmathematics results students related variables, teacher related variable andadministrator related variables were in consideration while analyzing and interpreting. Student's failure was strongly affected by all variables. Students themselveswere the key variables. All the variables related to students, teacher and administratorequally responsible for the students failure.Students are key, Teachers are guider andthe administrator is manager or facilitator for the student's success.
